Structured Medication Review (DES)

We have developed a self-assessment tool for Pharmacists to check if they have the right competencies to deliver reviews and this tool together with DES-SMR training programme will equip the Pharmacist with the right skills and knowledge to start delivering SMRs in your practice.

Our team of experienced Clinical Pharmacists has worked together with Danny Bartlett who is one of the first PCN pharmacists who currently works within West Sussex CCG and has trained and supported other PCN pharmacists within his CCG to deliver SMRs.

This programme consists of online videos, guides, assessments and downloadable GP system templates plus one-hour live drop-in session facilitated by Danny Bartlett.

During the drop-in session, Danny will be able to address any problems or questions the Pharmacists may have.

After this programme the Pharmacists should be able to:

-Understand the importance of the role of SMR in General Practice

-Be able to use our specific custom-built template that ensures the correct read-codes are entered

-Gather information from the patient efficiently

-Increase the clinical impact of the SMR

-Deliver SMRs safely and efficiently
