Clinical Pharmacist around Gloucestershire
to work in Primary Care
We are looking for a Clinical Pharmacist to join a friendly and supportive PCN in Gloucestershire. This is an exciting opportunity to gain a wide range of experience and make a real impact. The post holder will work as part of a multi-disciplinary team in a patient-facing role and clinically assess and treat patients using their expert knowledge of medicines. The post holder will also be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy. There will be support available for the right candidate to study towards an Independent Prescribing qualification.
Must be registered as a Pharmacist with the GPhC
📍Location: Gloucestershire
🗒Contract: Permanent
💷Salary: £40,057 - £45,839
⏰Hours: Part-time
✅Benefits: 5 weeks annual leave + BH, extra week of annual leave after 5 years of service and access to NHS pension
